Registration Process: If a new student is transferring to PHS, you must call for an appointment. (207-874-8255) You must bring @ time of appointment: · proof of residency (anything with your name and Portland address on it) · birth certificate · social security number · previous school transcript / medical records (Or if you do not have the records, you may fill out a Release Form from our office and the Guidance Office will send for the records - via fax. This needs to be done prior to the registration appointment.)
